Dear Parents/Students,
Please be informed that the Bally Municipality is conducting the second dose of Covid-19 vaccination programme for students of age group 12-14 (Born in the Year 1st January 2008 to 6th April 2010) scheduled on 10-05-2022 at Rose Bud School, Liluah. The Second Doze vaccination is exclusively for the students who have completed the First Dose vaccination from school.
Kindly find the consent google form link for Covid-19 vaccination at your registered WhatsApp No. with the school and confirm your presence.
You are further requested to register yourself at COWIN App following the below mentioned guidelines.
Students are requested to be present in the school at 11.00 A.M. on 10th May 2022 (Tuesday) Covid-19 Vaccination will start from 11:45 A.M.
Students are strictly advised not be on an empty stomach.
Guidelines for CoWin Portal:
Step
1: To register yourself on Cowin website go to https://selfregistration.cowin.gov.in/
Step 2: Sign for vaccination by entering your phone number in the given column
Step 3: The website will send you an OTP to verify, you can also login using your Aarogya Setu account.
Step 4: Tap on Register for the vaccine. Select your photo ID proof— it could be your Aadhar Card and other Government Photo Identity Card.
Step 5: Enter the Photo ID number, select your name and specify your gender and mention your year of birth.
Step 6: After filling in all the details, tap on Register.
Step 7: You will get a message on your registered number when you successfully register.
Step 8: Reference ID is generated. Please print the same.
Step 9: Do not forget to carry your Photo ID like Aadhar to the vaccination center and the print out of Reference ID.
It is important to Note that students must wear mask, use sanitizer and avoid gathering at covid-19 vaccination centre.
